Prospective Survey Data Dictionary
MVP COVID-19
The MVP COVID-19 Survey was developed for use by the Million Veteran Program. For this study, this survey is administered once at enrollment. Data include demographic information, a detailed symptom report, report of any testing and treatment, an evaluation of how COVID has impacted their daily lives, and non-COVID diagnoses and medication use. Data is stored in the REDCap schema, MVPCOVID19 table.
Flu-Related Symptoms Questionnaire
The Flu-Related Symptoms Questionnaire is collected at baseline and at up to 10 follow up study visits. Data include a detailed symptoms report (FluPRO®) and medication use. Repeat assessments are stored in a long format in the REDCap schema, FluPro table.
Vaccines Questionnaire
The Vaccinations Questionnaire is collected at baseline and at up to 10 follow up study visits. In each questionnaire, participants are asked to report if they have received any COVID-19 vaccinations since the last time the questionnaire was collected. Data are stored in a long format with one row per vaccine dose reported in the REDCap schema, Vaccines table.
Long-Term Symptoms Questionnaire
The Long-Term Symptoms Questionnaire is collected at up to 10 follow up study visits (i.e., not at the enrollment visit). Data include an evaluation of general health, functional status, report of symptoms, and report of recent testing and hospital admission. Repeat assessments are stored in a long format in the REDCap schema, LongTermSymptoms table.
Conditions and procedures
Both binary indications of the diagnosis of conditions and occurrence of procedures within a given number of days following the index date from the COVID Shared Data Resource and individual administrative codes indicating diagnosis or procedure with the date the code was applied in the EHR were included in the extract.
